A few more options to mail forms

Hi Oliver,

The mail form content part produces nice forms, but it would be great if we could:

1. Put fields in the same line. This could be done, maybe, by not changing the line at all between fields, unless the field description terminates with e.g. ";" (IT|name|1|Your Name|30,30||150|;)

2. put some extra text in the form (comments, instructions,...)
