I found the problem.
I did not check the field "CC to:".
But when you enter i.e the telephone field after "CC to:", then phpwcms hangs on it.

It actually gives the error:
The following From address failed: faerf
(when you activate the language file) :wink:

Strange that a userpanel canot be added.
You did go to article center -> edit article -> add content part?

Then you should be able to choose the right content part and see a list like this:
